| Precautions | Disease-related concerns:
• Renal impairment: Use with caution in patients with moderate-to-severe renal dysfunction and end-stage renal disease (ESRD) requiring hemodialysis; dosing adjustment required. Concurrent drug therapy issues: • High potential for interactions: Use with caution in patients taking strong CYP3A4/5 inhibitors (eg, atazanavir, clarithromycin, indinavir, itraconazole, nefazodone, nelfinavir, ritonavir, saquinavir, telithromycin; also see Drug Interactions); dosage adjustment of saxagliptin is required. • Insulin secretagogues: Use caution when used in conjunction with insulin secretagogues (eg, sulfonylureas); risk of hypoglycemia is increased. Monitor blood glucose closely; dosage adjustments of the insulin secretagogue may be necessary |
